Team Care Arrangements (TCA)
“Team Care Arrangements (TCA)” are comprehensive, collaborative programs to support patients with complex chronic health needs. The program is aimed at patients with a chronic health condition, which in this context is one which is present for at least 6 months, and who require a general practitioner and at least two other health professionals to work collaboratively to achieve desired health outcomes.
