General Practitioner Management Plan (GPMP)
A “General Practitioner Management Plan (GPMP)” is a comprehensive, individualised plan developed between a patient and their general practitioner to manage a chronic health condition. In this context, a ‘chronic health condition’ is defined as having been present for at least 6 months. The plan may include goals, tasks and actions to be taken to achieve the desired health outcomes.
