Domiciliary Medication Management Review (DMMR)
A Domiciliary Medication Management Review, or Home Medicine Review, is a comprehensive medication review carried out by an accredited pharmacist. The aim is to review and rationalise medications to ensure they are optimised to meet an individual's health goals.
